Qualifications
Full Time: 40 hours per week Mon-Fri 9:30am-6:00pm
Minimum Education Requirements: Masters Degree in Social Work
Minimum Experience Requirements: Previous Psychiatric Social Work experience required
Minimum Licensure/Certification: Licensed Master Social Worker in the state of Texas
Duties
1. Conduct psychosocial assessments to determine patient's level of functioning 2. Implement effective treatment 3. Plan appropriate discharge disposition 4. Conducts intake assessments and referral services 5. Evaluates patient safety at all levels of care 6. Assesses chemical use and history to plan appropriate level of care, interventions, and referrals 7. Provide didactic group therapy as scheduled and assigned 8. Adapts treatment to reflect age-specific and developmental considerations 9. Identifies realistic, appropriate, and individualized goals for patients care 10. Produces documentation that supports the treatment plan and meets regulatory requirements 11. Orients patients to programs and unit guidelines 12. Assists and educates patient and families with identifying appropriate discharge plans 13. Provides information on resources available 14. Collaborates with treatment team and physician to establish and facilitate discharge plan 15. Coordinates referrals and transfers with providers, community agencies, EAPs, payor source, and Springwood staff 16. Obtains discharge plan orders as indicated 17. Documents discharge as appropriate for program 18. Maintains and enhances social work skills 19. Serves as patient advocate 20. Recognizes and communicates ethical and legal concerns through established channels of communication 21. Offers assistance, preceptorship, and supervision to other staff and students 22. Maintains rapport and confidence of physicians 23. Adapts to changes in workload assignment to meet changing needs of the patients or programs 24. Perform other duties as assigned
