Qualifications
BASIC QUALIFICATIONS
Hours: 06:30 - 22:00, Sunday - Saturday (hours vary).
Minimum Education Required: B.S., Pharmacy. Pharm D. preferred.
Minimum Experience Required: No experience required, 1 year hospital experience preferred. Experience with Epic and Pyxis.
Licensure / Certification Required: Texas State Board of Pharmacy license.
Duties
1. Reviews medication orders for potential medication related problems. Intervenes as appropriate. Medication-related problems include: appropriateness of drug, dose, route, frequency, duration; minimizes therapeutic duplication; identifies/prevents drug-drug, drug-food, drug-lab, or drug-disease interactions. Prevents/reports adverse drug reactions (ADRs). Monitors patient responses to therapy.
2. Distribution/Technologies Verifies or performs accurate dispensing of pharmaceuticals. Processes medication orders Supports formulary process Monitors medication usage Educates and counsels patients regarding drug therapy. Interprets medication and medical literature for application to patient care.
3. Provides pharmacy technician supervision Assists with inventory control. Uses departmental technologies (eg epidural pumps, pharmacy information system, automated dispensing devices) Record keeping Communicates with electronic mail system.
4. Quality improvement Incorporates patient safety goals into daily patient care activities. Follows departmental policies and procedures. Looks for and suggests ways to improve. Implements improvements as appropriate. Participates in departmental quality improvement/drug use evaluation (DUE) activities.
5. Cost Effectiveness Uses time productively. Uses resources effectively. Complies with time and attendance policy. (Timeliness) Complies with time and attendance policy (Attendance) Suggests cost savings ideas.
6. Professional Activities Communicates with healthcare providers Assist in training other staff members or students. Formally shares educational information with healthcare providers in written or verbal format. Participates in professional pharmacy-related organization.
