How do I request a
Social Security number
for my newborn?

Parents may request a Social Security number for their newborn at the time of birth. The Social Security Administration will assign your newborn a number and mail the card directly to you.

The hospitals of Texas Health Resources facilitate only the initial issuance of birth certificates for newborns by governmental authorities. To obtain a birth certificate at a later date, please use the list below to contact the appropriate governmental agency.
