Diabetes Awareness Walk-a-Thon
Diabetes support group members, along with other members of the community, participate in the hospital's annual Diabetes Awareness Walk-a-Thon. Proceeds from the event support scholarships for the Diabetes Self-Management Education Program for those who can't afford to pay the program's enrollment fee.

Accessing the Program
To access the Diabetes Self-Management Education Program, ask your physician for a referral. The program is covered by Medicare and most insurance plans. Special arrangements can be made for those without health care coverage.

Once the referral is made, you will be contacted for an initial assessment appointment with the diabetes care team to determine your educational needs. For more information, call 1-877-THR-WELL (1-877-847-9355).
