Texas Health Azle Receives $420,000 Grant to Support Community Health Programs

AZLE, Texas — Texas Health Harris Methodist Hospital Azle has been awarded a $420,000 grant from the Texas Department of State Health Services to support community health programs that focus on wellness and disease prevention.

The Transforming Texas: Healthy People in Healthy Communities Grant aims to prevent heart attacks, strokes, cancer and other leading causes of death or disability in counties with populations of 499,999 or less through evidence and practice-based policy, environmental, and system change approaches. The grant funds received by Texas Health Azle will be used to collaborate with community organizations on community wellness programs in Parker and Hood counties.

The hospital plans to help educate residents on the importance of chronic disease self-management, smoking cessation, and healthy food choices. Chronic disease self-management education will aim to give patients the tools and resources to manage high blood pressure, high cholesterol, and diabetes symptoms and improve their quality of life.
