Texas Health Kaufman Offers Diabetes Care Program 09/05/2013
KAUFMAN, Texas — According to the Centers for Disease Control, approximately 25.8 million people in the U.S. live with diabetes. To assist in the management of this disease, Texas Health Presbyterian Hospital Kaufman is now offering a diabetes chronic care program.
These no-cost sessions will include self-assessment and goal-setting to determine individual needs as well as group visits and education by a pharmacist, clinical dietitian, certified diabetes educator and a physical therapist, among others. These medical professionals will focus on the following key areas of chronic disease management:

“Our goal is to provide support and assistance in self-care management to low income, uninsured and underinsured individuals with diabetes and pre-diabetes,” said Bette Akpan, director of quality and patient safety at Texas Health Kaufman.
