What is Population Health Management?

I’m Doug Hawthorne, CEO of Texas Health Resources, with “The Business of Health Care Report.”

I’d like to talk with you about “population health management.”

“Population health management” is a way of looking at the overall health needs of a defined group of people. For example, the population could be defined as Medicare patients in Tarrant County. Or it could be people who have diabetes and work for the State of Texas. There are many ways to define a population based on common factors.

New approaches to population health management offer some of the greatest opportunities to improve outcomes and — at the same time — help control the cost of care. Population health management means providing the right care at the right time in the right place, throughout the continuum of health needs.

Texas Health has begun initiatives with Methodist Health System, UT Southwestern Medical Center, and North Texas Specialty Physicians to design and implement new models of accountable care for defined populations.

We’re developing approaches that include:

  • Wellness and prevention
  • Primary and acute care
  • Rehabilitation, long term and hospice care

I look forward to sharing more with you on population health management and accountable care in the coming months.
