To be eligible, hospitals must complete at least 92 percent of birth registrations within five days of birth, hold staff training, track immunization consent forms, and maintain a state average completion of the Acknowledgement of Paternity form.
"Achieving this award became a reality thanks to the combined efforts of many individuals," said Becky Kulasevig, manager of Health Information Services (HIS). "It truly is a representation of teamwork between HIS, Siratt Women’s Center, Admissions, Social Services, department managers and the physicians on the medical staff.”
In addition to the Five-Star Award, Texas Health HEB will receive the Attorney General Award for the Acknowledgment of Paternity Completions and an award of excellence for Immtrac Texas Immunization Registry. All three awards will be presented in December at the annual Texas Vital Statistics Conference in Austin.
