Heart Failure Telemangement Research Project
Description
The purpose of this study is prevent heart failure patients from being re-admitted to the hospital. Once enrolled on the study, patients will call a toll-free number every day and answer a series of questions about their heart failure and enter their weight over the telephone. A nurse will monitor the responses and call the patient if they have any problems. The study will last 3 years.

Inclusion/Notes
INCLUSION:
Men and women 21 years or older.
Must have an admission for a diagnosis of heart failure within the previous 12 months.
Must be able to read and understand the consent form in English.
Must have daily access to a telephone.
EXCLUSION:
Patients under the age of 21.
Patients who are unable to read and understand an English consent form.
Patients who have been diagnosed with End Stage Renal Disease (ESRD), or are
receiving dialysis.
Patients who are under Hospice care.
Patients who permanently live in a skilled nursing facility.
Patients who are not physically/mentally able to self manage.
