Title	Bariatrics Outcomes Database (BOLD) (Weight Loss)
Description	This is a data collection and outcomes study of patients who undergo weight loss surgery.

Inclusion/Notes	INCLUSION: Patients who have signed an approved informed consent document. Patients which undergo bariatric surgery performed by a surgeon who has received Full Approval or Provisional Status designation in the American Society for Bariatric Surgery (ASBS) Bariatric Surgery Center of Excellence program, or by a surgeon who is a Fellow of the American Society for Bariatric Surgery. EXCLUSION: Patients who have not provided informed consent will be excluded. Patients who do not undergo bariatric surgery will be excluded.
