Title	Congested Heart Failure Telemanagement Pilot Study (Heart / Congested Heart Failure)
Description	This pilot study seeks to utilize new developments in telecommunications technology coupled with nursing protocol-based interventions (telemanagement) to reduce heart failure readmissions, length of stay and associated hospital costs.

Inclusion/Notes	INCLUSION: 21 years of age or older. Able to read and understand English. Must currently be hospitalized in a THR facility. Admitted for Acute HF or have a diagnosis of Acute HF during the current hospital admission. Weight =440 lbs (200 kg). EXCLUSION: Significant cancers or severe comorbidities (ESRD, Severe COPD, etc.). Survival expectancy <6 months. Being discharged to Hospice care, skilled nursing unit, continued care facility, nursing home or rehab facility. Hx of dementia, Alzheimer’s, etc. Unable to understand and follow directions.
