  • What to Bring to Your Appointment

    All patients should arrive 15 minutes prior to their appointment time. Please bring with you:

    • Photo ID
    • Insurance cards
    • Co-payments
    • New patient form (if applicable)
    • List of current medications

  • Office Policies

    Below you will find some of our office policies that are designed to make our patients feel at ease and provide you with the care that you deserve. We hope this information will be helpful and contribute to a better understanding between you and your physician. If there is anything we can do to make your visit more pleasant, please notify us.

    Office Hours

    Our office hours are Monday through Thursday 8:00 a.m. – 5:00 p.m. and Friday 8:00 a.m. – 12:00 p.m. Our office is closed 12:00 p.m. - 1:00 p.m. for lunch.

    Appointments

    We ask that you arrive for your appointments 15 minutes early for any paperwork that may need to be completed. We further suggest you schedule your follow-up appointment at the time of check out so you have a better selection of appointment times.

    Cancellations

    If you have a scheduled appointment time and need to reschedule or cancel, please notify our office at least 24 hours prior to your appointment. We will gladly reschedule you a more convenient appointment time.

    No Show Appointments

    If you fail to give 24 hours cancellation notice for a scheduled appointment, you may be charged a fee for that missed appointment. If you have special circumstances, please contact our office.

    Medication Refills

    Please have your pharmacy fax your medication refill request to our fax line (817) 348-0113. Please allow 24-48 hours for us to complete your request.

    Fees and Insurance

    Insurance co-payments, co-insurance and deductibles will be collected at the time of service. If you are a self-pay patient (uninsured), we require payment in full at the time of services. If you are unable to do this, please call our office to discuss payment arrangements.

    Privacy Practices

    Our practice is dedicated to maintaining the privacy of your health information. We are required by law to maintain the confidentiality of your health information.

    At Premier Dermatology, we understand patients want to have their office visit run as smoothly as possible. To help facilitate this process, we have provided our patient forms online for you to download and complete before your visit.

    Use the links below to find the patient form(s) you need, then click on the link to download, fill them out on your computer, print and bring them to your appointment. In order to protect your privacy, please do not email these forms.


  • Insurance Information

    We accept most insurance policies and will verify benefits prior to your appointment. However, we recommend you contact your individual insurance provider for benefit coverage.

  • Pay a Bill

    In order to use the payment portal to pay your bill, please have your statement and billing information readily accessible. For question or payments by phone, call us at 1-855-602-5273.
