Mammogram Myths vs. Facts: What Women Really Need to Know

If you’ve ever searched “When should I start getting a mammogram?” or “Do mammograms hurt?”, you know the internet is full of mixed messages. Some of it’s helpful, but a lot of myths persist online.

Misinformation about breast cancer screening can keep women from getting the care they need. Mammograms remain the gold standard for early breast cancer detection. Yet many women still delay or avoid screening because of confusing advice. And because one in eight American women will be diagnosed with breast cancer in her lifetime, misinformation isn’t just confusing — it can be dangerous.

We asked diagnostic radiologist Jill Chilcoat, M.D., a physician on the medical staff at Texas Health Harris Methodist Hospital Southwest Fort Worth, to share her insights as to what’s true and what’s not when it comes to mammograms. Here, we debunk some of the myths to help make it easier to protect your breast health.

Myth 1: Mammograms are Only for Women Over 50

Fact: Age 40 is the starting age for average-risk women, and earlier if you’re considered high‑risk.

This myth continues to stick around because mammography guidelines have changed over the years, and some conflicting guidelines remain in place. But the evidence is clear that starting annual mammograms at 40 saves lives.

While most cases of breast cancer do occur in middle-aged or older women, a breast cancer diagnosis can happen much earlier than age 50, says Chilcoat. About 10 percent of new breast cancer cases in the United States are found in women under age 45, and these cases are more likely to be found at a later stage, according to the Centers for Disease Control and Prevention (CDC).

“If you have a strong family history or other risk factors, such as a known genetic mutation, your doctor may recommend starting sooner,” Chilcoat notes. “Think of 40 as the ‘don’t wait past this point’ age.”

Myth 2: Mammograms are Painful and Unnecessary

Fact: They’re quick, tolerable, and are one of the most powerful tools for early detection of breast cancer or breast disease.

The compression from a mammogram lasts just a few seconds at a time, and the entire appointment is over before you know it. That brief discomfort means that a mammogram is doing a lot of heavy lifting by helping to detect breast cancer early, when treatment is most effective and least invasive. When caught early, the Breast Cancer Research Foundation reports that breast cancer is highly curable, often with fewer, less-intense treatments and surgeries. 

“It may not be the most relaxing 20 minutes of your life,” Chilcoat admits. “However, for most women it isn’t unbearable and can provide invaluable peace of mind. Myth 3: Mammograms Expose You to Radiation That Causes Cancer

Fact: The radiation from a mammogram is extremely low — about the same as a cross‑country plane ride.

“Modern mammograms use a very small amount of radiation that is well within safe limits regulated by organizations such as the American College of Radiology,” Chilcoat says. “The benefits of mammography far outweigh any risks.”

Myth 4: I Feel Fine, So I Don’t Need a Mammogram

Fact: Most early breast cancers don’t cause symptoms.

“Mammograms can detect changes long before you can feel anything, giving you more options and better outcomes if something is found,” Chilcoat says. “Mammograms matter because breast cancer can present with no lump, no pain, and no warning signs.”

Findings from the American Cancer Society show that early-stage breast cancer has a five-year survival rate of 99 percent. Breast cancers diagnosed in later-stages have a survival rate of 27 percent. That’s why screening is essential, even when you feel perfectly healthy.

Myth 5: I Can Skip Mammograms Because I Don’t Have a Family History of Breast Cancer

Fact: Most women diagnosed have no family history at all.

Only about 10 to 15 percent of breast cancers are caused by inherited mutations in genes like BRCA1 and BRCA2. It’s true that having a first- or second-degree relative with breast cancer can elevate your risk, but Chilcoat notes that family history is only one piece of the puzzle. Hormones, age, lifestyle, and sometimes pure chance all play a role. “So even if breast cancer doesn’t run in your family, screening still matters.”

Myth 6: A Negative Mammogram Means I’m in the Clear

Fact: A normal result is great news. But it’s not a lifetime guarantee.

“A mammogram is a snapshot of your breast health right now,” Chilcoat explains. “Breast tissue changes, and cancers can develop between screenings. That’s why yearly screening mammography is everything.”

Think of a mammogram as a tool for breast cancer detection, not prevention. Having a normal mammogram this year doesn’t mean that future mammograms will always be normal. Annual screenings help catch changes early and allow the earliest possible cancer diagnosis.

Myth 7: A 3-D Mammogram is the Same as a Traditional Mammogram

Fact: Three-dimensional mammography (also called tomosynthesis) takes multiple images in thin tomographic “slices,” giving radiologists a more complete picture of your breast tissue.

“A 3-D mammogram provides far greater detail than a 2-D mammogram and can help distinguish between overlapping normal breast tissue and cancer,” says Chilcoat. “Tomosynthesis, or 3-D imaging, is now routinely performed by physicians on the medical staff for most screening studies because of known benefits.”
