This is the sixth hospital in the system to offer this service to patients

ARLINGTON, Texas — Texas Health Care at Home, an innovative model that brings hospital level clinical care directly into patient homes, is now available to qualifying patients within a 25-mile radius of Texas Health Harris Methodist Hospital Southlake. Care at Home was first launched by Texas Health in 2022.

“We are partnering with consumers to provide a lifetime of health and well-being, and that means thinking beyond traditional healthcare models,” said Kathi Cox, Ambulatory and Virtual Channel chief operating officer. “We are supporting patients wherever they are.”

Texas Health Southlake is the sixth hospital in the system to offer this service to patients needing medical care who meet certain criteria. It is available to patients with Medicare and Medicare Advantage plans who are diagnosed with conditions such as, but not limited to, heart failure, chronic obstructive pulmonary disease (COPD), asthma, pneumonia, cellulitis/soft tissue infections and complicated urinary tract infections.

Patients are cared for at home by a mix of providers who work in unison with healthcare professionals stationed at a virtual medical command center.

Care at Home is also offered at Texas Health Harris Methodist Hospital Alliance, Texas Health Harris Methodist Hospital Fort Worth, Texas Health Harris Methodist Hospital Southwest Fort Worth, Texas Health Harris Methodist Hospital Hurst-Euless-Bedford and Texas Health Presbyterian Hospital Plano.

The service has grown to cover a large portion of the Dallas region because each hospital offers the program to patients within a 25-mile radius. The service is currently bounded by Celina to the north, Wylie to the east, Cleburne to the South and Hudson Oaks to the west.

Texas Health continues to integrate advanced technology amid changing consumer demands, with an eye toward emerging health industry trends.

“We are transforming the way we provide healthcare,” said Mae Centeno, D.N.P., APRN, ACNS-BC, chief operating officer/chief nursing officer, Virtual Care Delivery. “This expansion to the Southlake community brings an innovative care model to more patients.”

The multispecialty hospital has served Southlake and Northeast Tarrant County for more than 20 years.

“We are excited to offer Texas Health Care at Home to our patients,” said Traci Bernard, R.N., president of Texas Health Southlake. “We are continually working to deliver high-quality care and exceptional experiences at every consumer interaction.”
