Appointment Info

Before using the center, patients must obtain a written prescription from their physician directing the course of therapy, except for participation in some of the community programs. For more information on how to register for one of the community programs, please click on the specific program of interest at right.

Patients coming to the rehabilitation center for their first appointment are required to complete a medical history form. Follow this link to download and print the form.

The Physical Medicine and Rehabilitation Center is open from 7 a.m. to 6 p.m. Monday through Friday.
