Texas Health Community Hope is a comprehensive initiative by Texas Health Resources aimed at addressing the root causes of health disparities and promoting holistic well-being across North Texas. By focusing on vulnerable communities, the program strives to ensure that where one lives does not determine their health outcomes.

Community Health & Well-Being Programs

Texas Health Community Hope offers a range of innovative programs designed to improve the health and well-being of North Texans:

  • Wellness for Life® Mobile Health: A mobile health program that delivers healthcare services directly to communities who may lack access to care.
  • THRIVE: This program aims to improve the mental and physical health of at-risk students and their families by providing access to mental health resources, healthy food, and nutritional education.
  • Texas Health Community Impact (THCI): Awards grants to local organizations that work collaboratively to tackle health disparities and socioeconomic hardships.
  • Community Resource Center (CMRC): The Community Resource Center at Texas Health Presbyterian Hospital Allen is a multi-faceted intervention that improves health outcomes, fosters community connectivity, and empowers and educates Collin County residents.
  • Health to Home: Provides clinical support and housing solutions to adults experiencing homelessness, aiming to improve recovery and health outcomes.
  • Forensic Healthcare & Violence Intervention Program: Offers specialized care for victims of sexual assault, including 24/7 availability in emergency departments, expert testimony in legal proceedings, access to organizations and resources that can help, and community education on topics like human trafficking and child abuse.
  • Maternal Health Project: Texas Health aims to elevate maternal health care by providing blood pressure cuffs who are at-risk of pre-eclampsia. The blood pressure cuffs allow at-risk mothers to monitor their blood pressure in between physician visits both while pregnant and after delivery.
  • Good For You Healthy Hubs: These hubs provide fresh fruits and vegetables to the community, focusing on areas with limited access to healthy food options. The program began in 2019 and has grown to more than 20 locations, including elementary and middle schools and community centers. In 2023, the hubs served over 139,000 people and distributed nearly 805,000 pounds of fresh produce.
  • Double Up Food Bucks: This national incentive program aims to close the nutrition gap for low-income families by increasing purchasing power for fresh fruits and vegetables. It allows individuals who qualify for Supplemental Nutrition Assistance Program (SNAP) benefits to double their buying power for fresh produce at participating locations.
  • School Wellness Network: Supports schools in creating environments that promote healthy behaviors through programs like learning gardens and nutrition education.
  • Mental Health First Aid: Provides training to recognize and assist individuals experiencing mental health or substance abuse crises.
