Texas Health Community Impact

Every person deserves to live in a community that supports healthy choices. However, according to the Centers for Disease Control and Prevention (2023), certain ZIP codes are at a significant disadvantage. Texas Health Community Hope is Texas Health Resources’ unique approach to promoting healthier futures through a broad range of impactful initiatives, investments and collaborations.

As a department of Texas Health Community Hope, Texas Health Community Impact invests in community health and well-being through grantmaking and strategic collaborations. From data-driven, home-grown regional strategies to technical assistance, Texas Health Community Impact collaborates with community leaders to offer creative grant opportunities to North Texas organizations with a mutual desire to transform health and well-being in innovative ways.

Impact Across North Texas Counties

Spanning nine counties, Texas Health Community Impact leverages a regional approach for data-driven funding strategies and grant opportunities. Governed by five Leadership Councils made up of community volunteers, each region is positioned to develop their strategy with a clear understanding of community needs and priorities unique to its area.

Our funding priorities are guided by Community Health Needs Assessments (CHNAs) as a foundation, in addition to other data sources—like the U.S. Census Bureau and Healthy North Texas. We use these trusted data sources to help us fully understand the intersection of health and non-medical drivers of health, plus their impact on health outcomes.

Successful Collaborations

Driven to catalyze change at the local level, Texas Health has allocated $23.2 million in Texas Health Community Impact grants to North Texas organizations since 2019. These grants fund bold initiatives led by community collaborations to address mental, physical, emotional and spiritual well-being in our communities of greatest need.
