  • Patient Forms

    At Denton Digestive Care, we understand you want to have your office visit run as smoothly as possible. To help facilitate this process, we request that you fill out your patient information prior to your visit. Our office provides each new patient an activation code to sign up in MyChart and submit information online. Please contact our office for additional information or to obtain your activation code.

    Simply click on the links below to download. Print, complete, and bring the forms to your appointment. In order to protect your privacy, please do not e-mail forms.


  • Insurance Information

    We believe in making it easy for our patients to focus on their treatment and regaining good health. Denton Digestive Care accepts a wide range of insurance plans and various forms of payment, and will verify benefits prior to your appointment.   • Pay a Bill

    In order to use the payment portal to pay your bill, please have your statement and billing information readily accessible. For question or payments by phone, call us at 1-855-602-5273.

  • Office Policies

    At Denton Digestive Care, it is our desire to provide you with timely, quality care. In order to do so, please arrive on time and to make every scheduled appointment. We feel it is important for you to understand our policy as it relates to cancellations, no shows and late arrivals.

    New Patients

    Please be sure that you have completed the online New Patient Forms. If not, please arrive 30-45 minutes early to complete the information. This early arrival allows our staff to efficiently perform its task of registering you into our systems.

    Follow-Up Visits

    For follow-up visits, we ask that you arrive on time. Should you arrive more than 15 minutes late, your appointment may be moved to a work-in status or rescheduled.

    Emergencies

    All emergencies are seen within a timely fashion, based upon the urgency of your problem. In a true emergency, it is best to dial 911 or go to the emergency room at your nearest hospital. The emergency room staff will contact us or the physician on call.

  • Support Groups

    In addition to providing compassionate care, our goal is to offer education and support to help you improve your digestive health. Support groups can be very helpful in times of difficulty, as they allow our patients to communicate with others who share a similar condition.     We encourage caregivers and significant others to join patients in utilizing support services. Our staff will help with referral assistance to community resources, so you can make the connections you need.

    Our team continually assesses further approaches to treatment for digestive diseases and disorders, in great part with input from those who matter most — our patients.
