Mobile HELP
(Healthy Education Lifestyle Program)
What is Mobile HELP?
Mobile HELP serves persons who have been diagnosed with diabetes, high blood pressure, and/or high cholesterol. A physical exam, blood tests, and learning aides are provided at no charge. Medicine prescriptions are selected to provide the best care at lower costs.
Mobile HELP also serves persons who are at risk for diabetes, high blood pressure, and/or high cholesterol. A physical exam, blood tests, and learning aides will be provided at no-charge. If medication is needed, it will be selected to provide the best care at lower costs.
To Qualify:
- Be age 21 year or older (no upper age limit)
- Currently have no health insurance for outpatient care services
Mobile HELP Staff will provide:
- Office visits provided at no charge on a mobile unit that visits your community
- Physical examinations and blood tests will be provided at no charge
- Medicine prescriptions will be sent to your chosen pharmacy, always mindful of lower cost
- Individual and group education will be made available in your language
- (Full-service primary care is not available on the mobile unit)
Patient’s Responsibility:
- Attend your scheduled appointments (and be mindful of others, cancel when you can’t make it)
- Be willing to make lifestyle changes that improve your health
- Be sure to bring any medical records, blood sugar and blood pressure diary
