According to the Centers for Disease Control and Prevention, approximately 25.8 million people in the U.S. live with diabetes. To assist in the management of this disease, Texas Health Presbyterian Hospital Kaufman is now offering a diabetes chronic care program.

Each no-cost session will include self-assessment and goal-setting to determine individual needs as well as group visits and education by a pharmacist, clinical dietitian, certified diabetes educator and a physical therapist, among others.

These medical professionals will focus on the following key areas of chronic disease management:

  • Disease process
  • Medications
  • Self-management
  • Diet
  • Complications and risk factors
  • Exercise
  • Emotional support
