  • Insurance Plans

    Texas Health Medical Associates accepts a variety of health insurance plans. Insurances accepted may vary from physician to physician and are subject to change without notice. Please check with the office you wish to visit before your appointment to ensure coverage.

  • Cancellations and Missed Appointments

    Texas Health Medical Associates reserves your office appointment time slot exclusively for you. As a courtesy to our staff and other patients needing to be seen by our physicians, we request at least 24 hours’ notice for cancellations and/or rescheduling of an appointment.

  • Pharmacy Refills

    To assist in getting your medications refilled in a timely fashion, please submit your requests for prescription refills directly to your pharmacist. The pharmacy will then fax over a refill request, and once authorized by your physician, the prescription will be called into your pharmacy to be filled.

    To avoid going without your medications and last minute requests, please anticipate the need for refills and give your pharmacist approximately 48-72 hours advance notice.

    Please note that some refills may require the patient to first be seen by a physician.

  • Lab Work

    The Texas Health Huguley Outpatient Laboratory, located in suite 122 of Medical Office Building 2, can perform any lab work ordered by Texas Health Medical Associates physicians.

    For the convenience of patients at off-campus clinics, those offices offer on-site lab draws. Specimens taken are promptly sent off to the Texas Health Huguley Outpatient Lab and the results are returned to the ordering physician in a reasonable time period to aid in your diagnosis, treatment, and care.

  • Imaging Services

    To schedule imaging services, please call STAR Scheduling at 817-551-2787

    You are required to bring a physician order for imaging services, a current ID, and insurance card with you to your appointment.

    The Texas Health Huguley Imaging Center performs the following services:

    • General X-Ray
    • CT Scan
    • Digital Mammography
    • Ultrasound
    • DEXA – Bone Densitometry
    • Stereotactic Breast Biopsy

  • Request Medical Records

    To request a copy of your medical records, download the Request for Access to PHI Form.

    To request a copy of your medical records to be released to another healthcare provider, download the Authorization for Access, Use and/or Disclosure of Protected Health Information Form.

    After you print and complete the form, you may either:

    • Bring it directly to the office
    • Fax it to your physician's office
    • Or drop it in the mail

    Once the form is received, we will then release your records accordingly.

    Medical records are the property of Texas Health Medical Associates. Records are transferred to another physician as a courtesy at no cost. A personal copy of your medical records will cost $25 for the first 50 pages, and $.50 per page thereafter.

  • Bilingual Services

    For those patients who are more comfortable with a doctor that speaks Spanish, several THMA physicians are available to assist you. A translation service or a sign language service can be provided for patients as needed.
