Employees and dependents age 18+ who are covered on a Cook Children’s medical plan are eligible to use THPG for comprehensive medical services. If covered on the HRA medical plan, there is no cost for services. If covered on the HSA medical plan, there is a $10 copay for a non-preventive visit, and no cost for a preventive visit. Employees not enrolled on the Cook Children’s medical plan are eligible to use THPG for acute care (examples: flu symptoms, earache, upper respiratory) and will pay a $10 copay. Family members of employees not on the medical plan are not eligible for this benefit, however they can utilize any THPG providers at the listed branch locations with their private insurance.
