The Healthy Education Lifestyles Program (HELP) can assist you by providing clinical care, resources and education. HELP assists uninsured patients in managing chronic conditions such as diabetes, high cholesterol and hypertension.

HELP uses a coordinated, team-based approach where patients can build a relationship with the HELP staff. Every HELP visit is comprised of three key components — an individual visit with a mid‐level practitioner, including necessary lab testing; an education session to increase health literacy; and a support group, which is crucial for establishing and ensuring accountability for improved health outcomes.

It provides program participants with ongoing health coaching and education resources to support patients learning to effectively manage their chronic disease and encourage them to take an active role in reducing the negative effects their chronic conditions will otherwise take on their lives. Additionally, HELP staff work with patients to reduce the barriers often encountered to self-managing their disease (access to healthy food, access to primary care, lack of transportation, etc.).

    The Healthy Education Lifestyles Program (HELP) outpatient program is designed to assist uninsured individuals with chronic disease management through encouraging lifestyle changes, while also providing access to affordable care. Patients receive an individual visit with an Advanced Practice Provider, including necessary lab testing; and an education session to increase health literacy, which is crucial for establishing and ensuring accountability for improved health. All of this is provided for a small $10 copay per visit. Program is open on Mondays and Thursdays.

    Qualifications

    Patients accepted into HELP are between 18-64 years old, are currently uninsured and have been diagnosed with one or more of the following chronic diseases (included but not limited too):
    Diabetes Mellitus (Type I or II)
    Hypertension/High Blood Pressure
    High Cholesterol
    Congestive Heart Failure
    Patients with end stage renal disease or enrolled in hospice are not eligible.

    Patient Responsibility

    • Attend HELP sessions as recommended.
    • $10 copay per month
    • Conscious effort to make lifestyle changes to improve the management of any and all chronic diseases

    The Healthy Education Lifestyles Program (HELP) outpatient program is designed to assist uninsured individuals with chronic disease management through encouraging lifestyle changes, while also providing access to affordable care. Patients receive an individual visit with a mid-level practitioner, including necessary lab testing; an education session to increase health literacy; and a support group, which is crucial for establishing and ensuring accountability for improved health. All of this is provided for a small $20 copay per visit. Program is open Tuesday, Thursday & Friday 7am-4:30pm.

    In addition, the following services are provided to all HELP program patients:

    • Access to a Licensed Social Worker for referrals and access to community resources
    • Access to Chronic Disease Self-Management Workshops (Stanford programs)
    • Referrals for low-cost annual comprehensive eye exam
    • Prescription Assistance
    • Mental Health counseling

    Qualifications

    Patients accepted into HELP are between 18-64 years old, are currently uninsured and have been diagnosed with one or more of the following chronic diseases:
    Diabetes Mellitus (Type I or II)
    Hypertension/High Blood Pressure
    High Cholesterol
    Congestive Heart Failure

Patients receive an individual visit with a mid-level practitioner, including necessary lab testing; an education session to increase health literacy; and a support group, which is crucial for establishing and ensuring accountability for improved health. All of this is provided for a small $10 copay per visit. Program is open Monday - Friday.


    Qualifications

    Patients accepted into HELP are 18 and over, are currently uninsured and have been diagnosed with one or more of the following chronic diseases:
    Diabetes Mellitus (Type I or II)
    Hypertension/High Blood Pressure
    High Cholesterol
    Congestive Heart Failure
    COPD
    Asthma
    Patients with end stage renal disease or enrolled in hospice are not eligible.

    Patient Responsibility

    The Healthy Education Lifestyles Program (HELP) outpatient program is designed to assist uninsured individuals with chronic disease management through encouraging lifestyle changes, while also providing access to affordable care. Patients receive an individual visit with an Advanced practice provider or Nurse Practitioner, including necessary lab testing; an education session to increase health literacy; and a support group, which is crucial for establishing and ensuring accountability for improved health. All of this is provided for a small $10 copay per visit. The program is open Monday, Wednesday, Thursday & Friday.
    In addition, the following services are provided to all HELP program patients:

    • Primary Care
    • Access to an RN Diabetes Educator
    • Prescription Assistance

    Qualifications

    Patients accepted into HELP are between 18-64 years old, are currently uninsured and have been diagnosed with one or more of the following chronic diseases:
    Diabetes Mellitus (Type I or II)
    Hypertension/High Blood Pressure
    High Cholesterol
    Congestive Heart Failure

    Not Eligible

    Patients with end stage renal disease or enrolled in hospice
    Pregnancy, unstable/unmanaged mental health & illegal substance abuse

    Patient Responsibility

    • Attend HELP sessions as recommended by program staff
    • $10 copay per month
    • Conscious effort to make lifestyle changes to improve the management of any and all chronic diseases

Texas Health will be integrating HELP with the Mobile Health Program. HELP on Wheels will provide the same level of services as the hospital-based programs but housed on a mobile health unit brought to the community.
