Outpatient Rehabilitation Program
The cardiac rehabilitation program consists of
three phases. Patients are evaluated by the cardiac
rehabilitation staff and, depending on their stage of treatment,
are assigned to the appropriate phase.
PHASE I - Phase I was developed for heart and
vascular patients who are currently in the hospital. Phase I
provides patients with educational materials and a registered
nurse who is available to consult and answer any questions
about continuing with the program.
PHASE II - Phase II provides exercise sessions
three times a week over a 12-week period with full
Electrocardiogram (ECG) monitoring. Each session is tailored to
meet the individual needs of each patient. Education classes
are also offered on a variety of topics that include: exercise,
nutrition, stress management and more. Both patients and their
family members are encouraged to attend. Note: Phase II is
covered by most insurance plans.
PHASE III - Phase III is designed as a
maintenance program for Phase II graduates. The program allows
patients to continue on their exercise program under the
supervision of the Cardiac Rehabilitation staff. The
maintenance program is also available for individuals at high
risk for cardiovascular disease. Note: Phase III is not
covered by insurance plans. The cost is $50 per month.
Phases II and III Cardiac Rehabilitation sessions are held at the
