What is the Well-Being 5?

The Well-Being 5 is a highly personalized scientific survey you complete online that measures your current well-being and gives you a snapshot of it along with the five elements that affect your well-being. The Well-Being 5 also gives you insights, recommendations and access to resources that can empower you to reach higher well-being.

Who can take the Well-Being 5?
You must live in Texas Health Resources' service area and be at least 18 years old to participate. Any adult will benefit from finding their well-being score by taking the Well-Being 5.

Do I need to finish the entire assessment in order?
No. You can skip a question and return to it later. However, the more information we have, the more accurately we can find your well-being score.

What happens during my Well-Being 5?
1. You’ll complete the Well-Being 5 survey by responding to easy, concise questions and statements about your sense of purpose, social relationships, financial security, your relationship to your community and your physical health.
2. You’ll get your Well-Being 5 snapshot online, which includes your Well-Being 5 score and feedback you can act on right away. You will also have the option to print or download it.

How do I complete my Well-Being 5?
1. Visit YourWellBeingScore.com from your computer, tablet or smartphone.
2. Complete the registration form.
3. Check your Inbox for a confirmation email.
4. Follow the link in the email to your personal Well-Being 5 assessment.

What do I do if my Well-Being 5 link has expired?
If your link has expired, go back to your original email and follow the link again. Then follow the steps on-screen to request a new link to access your Well-Being 5 assessment.

What do I do if I lost my Well-Being 5 email?
Click here and register again using the same information as before, then check your inbox for a confirmation email that will contain a new link to access your Well-Being 5 assessment.

Most people complete the Well-Being 5 in 10–15 minutes; however, you can save your information and pick up where you left off at another time if needed.

What is the average Well-Being Score?
According to the 2014 Gallup-Healthways Well-being Index, the average overall well-being score for people in the United States is 61.6. Women score an average of 62.4, and men average 60.8.

Why am I getting these resources, and are they free?
Texas Health Resources and Gallup-Healthways are offering the Well-Being 5 survey to all North Texas residents at no cost. By participating, you receive valuable information about your overall well-being as well as resources and support to improve and maintain it. With higher well-being, you could experience a better quality of life and performance as well as lower health-related costs.

Should I participate even if I’m healthy?
Yes. Because you may not experience symptoms for some health risks, you could be at risk without even knowing it. Plus, participating will help you better understand your current well-being overall (more than just your physical health) and take action on your goals.

Can I participate if I’m under a doctor’s care?
Yes. This program can help support your doctor’s care plan. However, the information and support you receive through the Well-Being 5 is not intended to replace your doctor’s medical care or advice.
